Where can I get tax forms?

Federal tax forms and publications, including previous years' tax forms, may be obtained online at the Internal Revenue Service's (IRS) website. You can order many of the current year tax forms and publications for delivery by U.S. mail. Be aware that not all tax products are available through the website and quantities are limited. You may also wish to contact the IRS Forms and Publications Hotline at 1.800.829.3676 between 7 a.m. and 10 p.m. your local time, Monday through Friday.

Most Requested Forms (the following forms are all .PDF documents)

  • Form W-4 - Employee’s Withholding Allowance Certificate
    • Used by your employer to withhold the correct federal income tax from your pay. Because your tax situation may change, you may want to use the IRS's Withholding Calculator to help you determine your correct federal withholding tax each year.
  • Form W-9 - Request for Taxpayer Identification and Certification
  • Form 1040 - U.S. Individual Income Tax Return
  • Form SS-4 - Application for Employer Identification Number
    • For use by employers, corporations, partnerships, trusts, estates, churches, government agencies, Indian tribal entities, certain individuals and others.
  • Form 1040X - Amended U.S. Individual Income Tax Return
